You have set up your survey within your email notification template within Freshdesk which is displaying correctly within Freshdesk itself.
However, when the email is triggered, it is received in the email inbox completely blank without the survey or body text.
This could be due to the line length exceeding beyond the permissible RFC standards due to which the mail is being sent as blank. This is usually caused by the presence of too many div tags or other tags contributing to the length. Modifying those or the length of the body text should resolve the issue.
